Our Case Review was specifically created for those suffering from a chronic illness or for those with a more complicated case history. These are patients who have been bounced from doctor to doctor and still have no answers or improvements in quality of life.

The way that Dr. Hagmeyer works with patients differs considerably from the conventional medical model you have been accustomed to. Our office focuses on Holistic and Functional Medicine. Dr Hagmeyer does not prescribe medication, nor are we to take the place of your Primary Care physician. Instead, we will focus on the root cause of your health problems by implementing a Holistic Functional Medicine approach.
Functional Medicine looks at how to nurture your body back to health with diet, nutritional supplementation, lifestyle changes and education. It is empowering the patient.

THE CASE REVIEW
The Case Review is the first step for all new patients with complex health problems. This is where Dr Hagmeyer will really dig into your health story. This is where all the details of your life matter! This is your chance to share your life-story and give Dr Hagmeyer a glimpse into your daily struggles, and stressors.
We ask that perspective patients type up a health narrative based on specific questions we have put together to help guide you. Ultimately, this is your story and your opportunity to tell Dr Hagmeyer everything you want him to know about your health.
By reviewing your past testing, dissecting your health history, reviewing a 7 day food diary, Metabolic Assessment questionnaires, Neurological Assessment questionnaires, Dr Hagmeyer will start connecting the dots and determine the most appropriate lab testing to uncover where your problems are rooted.
The Case Review is designed to:
- Save You Money and Time in the long-term by performing the RIGHT diagnostic testing right out of the starting gate. This way we can see how the missing health pieces of your health fit together and speed up the healing process.
- Identify specific metabolic imbalances and best way to approach these imbalances.
- Determine if our office is a good fit for you over the long-term.
- Determine Cost of Testing or Treatment Program.
- It gives you the chance to ask any questions you may have about your condition and what you should expect.
Your Case Review Is A Two Step Process:
- STEP ONE- CASE REVIEW : INITIAL REVIEW OF RECORDS
This initial 60-90 minute appointment is designed to make sure that we have all of the information needed to gain a comprehensive understanding of the many factors affecting your health.
As part of the Case Review, a New Patient Coordinator will provide you with the health history paperwork to complete. It is important that you spend time and energy completing these intake forms, as they become the cornerstone of future testing and treatment.
Your New Patient Application paperwork includes:
- A detailed health and medical history questionnaire
- A questionnaire of your chief complaints and most important health goals
- An assessment of your most troubling and frequently experienced symptoms
- A 7 day diet questionnaire (food Journal)
- A survey of your current supplements and medications
- Metabolic Assessment Questionnaire and
- Neurological functional Screening questionnaire.
Note: We do feel that it is important for us to review previous labs and records. Please provide and submit any blood work and testing (last 6 months) to our office with your New patient application. Dr. Hagmeyer will review these at the time of your Case Review. Dr Hagmeyer only accepts a limited number of patients per month into the signature program. This ensures that our patients get the quality of care they deserve and the time they need.
STEP TWO OF THE CASE REVIEW: REPORT OF FINDINGS
After Dr Hagmeyer has familiarized himself with your New Patient Application, Health Records, Past Testings, Metabolic Assessment forms, Neurological Assessment form, Health Narrative, and 7 day food diary, Dr. Hagmeyer will prepare a report for you.
At the Report of Findings visit, Dr Hagmeyer will:
- Review your past test results and metabolic patterns and what was discovered or missed by past doctors.
- Review the recommend Functional laboratory Testing, specifically tailored to your personal case and your health goals.
- Review the costs of testing and or treatment plan.
Step Two or “Report of Findings” visit is a 60-90 minute appointment with Dr Hagmeyer. It is important that you have access to a computer for this consult because Dr Hagmeyer will be using a screen sharing program- that enables you to see Dr Hagmeyer’s computer screen and report.
How Much Does The Case Review Cost?
- Review of your Records prior to your consult; detailed health and medical history questionnaire, an assessment of your most troubling and frequently experienced symptoms, 7 day food Journal, survey of your current supplements and medications, Metabolic Assessment Questionnaire, Neurological functional Screening questionnaire. The Cost to review this information is $395
- Your Reportof Findings- 60 minute consult with Dr Hagmeyer to discuss these results, discuss recommended testing $195
- The Total for the case Review (Records review and Report of findings) is $595.
On average, the total you should expect to pay for the entire Case Review process including Initial Review of Records, Follow up Visit, Report of Findings (60-90 minutes), and discussion of Recommended Lab testing with Dr Hagmeyer is $595. There are no additional or hidden fees.
What kinds of Tests will Dr Hagmeyer Recommend at My Report of Findings
This depends on your needs, goals and the kind of help you are looking for from Dr Hagmeyer. Due to customized nature of the Signature program, each patient and the testing that a patient needs varies. Dr Hagmeyer does not practice cook book medicine nor does he have a cookie cutter approach to testing.

IS TESTING AND TREATMENT WITH DR. HAGMEYER AFFORDABLE?
We are very sensitive to our patients needs and understand that the majority of testing needed in chronic complex cases is not covered by insurance. This is why we spend the time we do during the initial case review process to understand your needs. By understanding your case from the very beginning, we can eliminate any unnecessary testing, saving you time and money.
Every case is different and we do our best to limit testing to the areas that will provide the greatest amount of objective data. A successful treatment plan that focuses on getting to the root cause, will be dependent on correct testing or data. Without testing- we are guessing.
Complex, Chronic, long standing cases require in depth functional testing. Insurance companies fail to recognize this as the testing that predominates in our health care system is for the purpose of providing drug based or surgical treatment.
We suggest that ALL perspective patients fill out a short questionnaire that will help Dr Hagmeyer understand your top Health problems, budget for testing and the kind of help you are looking for.
If Dr Hagmeyer does not feel your budget would accommodate your level of recommended testing, given your health concerns and problems, you will receive an email notifying you of Dr Hagmeyer’s next best recommendations.

I’m Ready To Get Well. Let’s Get Started
Question– Will insurance cover my testing?
Answer– Yes and No. Basic blood work such as (Thyroid levels, Cholesterol, Blood sugar, etc) can be billed to Labcorp or Quest. Functional Advanced laboratory testing is not covered by insurance. Chances are if you have already been from one traditional doctor to the next, without answers, then you will need Functional Laboratory testing.
Question– My case is more complicated, What is the typical out of pocket investment for Functional testing
Answer– Testing can range anywhere from $2,000-$4,000 depending on the complexity of your health history and how in depth Dr Hagmeyer feels he needs to get in order to develop the appropriate program. Advanced laboratory testing (blood, stool, saliva, urine, breath) is purchased by our office to the respective labs.
Once these test kits are purchased, they are then mailed out to you. If the cost of functional testing is out of your health budget, this is not the correct program for you at this time.
We suggest you contact our office and schedule a 30 minute consult or consider one of the other condition specific programs. These “condition specific programs” range from $500-$1,000.
Question– Can I submit Testing to my insurance company.
Answer– Yes- Once we have received your test results, We will do our best to code them appropriately and help you with reimbursement-but we make no guarantee of payment. We charge $20 for coding and preparation of these forms.
Question-Do you offer payment plans on the testing?
Answer– For patients who prefer to finance their testing, we now offer Advance Care. This card has no annual fee and 0% interest if paid off within the promotional time period. Feel free to explore their site and email us with any questions.
Question– Can I split up my Payment For Testing?
Answer– Yes- We can break the payment up so that a portion of tests are purchased in the first 30 days, and the remainder of the tests ordered 30 days out. Beyond that, we recommend exploring Advance care.
